This program will help employers upskill crews, improve quality, and stay ahead of demand, while giving employees a chance to advance quickly and stay competitive.

The purpose of this form is to acquire consent for providing your information to properly administer the Training for Residential Energy Contractors grant (Award No. DE-SE0000796.0000). This grant was awarded to the PA Department of Environmental Protection by the US Department of Energy.

By signing below, I authorize Penn Heat Pump Pathways and its program administrators, including Home Performance Coalition (HPC), TixStone Solutions, CareerEquity (CE), the Pennsylvania Department of Environmental Protection, Fierce Creative Agency, Research for the Future, training providers, and funding or audit entities, to collect program-related information necessary for eligibility determination, enrollment, reimbursement, reporting, compliance tracking, and program administration. This includes, but is not limited to, name, date of birth, address, employer name, attendance records, completion status, certification outcomes, assessment results, employment placement information, support service participation, and any documentation required to administer training funds or reimburse providers.

I understand that disclosure of this information is a required condition of participation in the Penn Heat Pump Pathways Program.
